A message from One Education about the Sandwell Well-being Charter Mark

Your child’s school is working on the Sandwell Well-being Charter Mark. As part of this project, we are asking pupils to complete a brief online questionnaire in their class, looking at how they feel about themselves, their friends and school. This will be carried out on two separate occasions and takes around 10 minutes to complete. The results will be used to help the school develop an action plan to promote well-being. Your individual child will only be known to us by a number which will be allocated by the school (This will be their catering number). The survey will ask 12 Statements, which your child(ren) can answer: Some example statements are given below:

“I can cheer myself up”
“I can listen to others”
“I like learning new things at school”

The results will be given to the school in a report. One Education will not know any identifying details about your child other than a catering number. We will keep other information collected in line with One Education Data Protection Policy. As One Education, Educational Psychology are analysing anonymous data, you have the option to reply to this letter to opt-out, if you do not consent to your child’s participation.

If your child changes their mind about answering the survey after they have completed it, please let the school know and their information can be removed.

If you have any questions please contact one of the team members on the email address amy.burns@oneeducation.co.uk. Alternatively, you can contact One Education, Educational Psychology on (08449671111)

Kind Regards,

Dr Amy Burns (Educational Psychologist at One Education)

If you do not want your child to take part in the survey please complete this form.

We wish you all a peaceful and relaxing break.
